How to determine your eligibility:

Verification of Employment: Choose this if you are a parent/guardian that is currently working. If you are paid by check, please provide us with the four (4) most recent paystubs.

 

Self-Certification of Employment: Choose this if you are a parent/guardian that is paid in cash. You will still need to fill out the Verification of Employment.

 

Seeking Employment: Choose this if you are a parent/guardian that is currently not working.

 

Training Verification: Choose this if you are a parent/guardian that is currently going to school (college/university, trade school, English proficiency classes, etc.)

 

At Risk Referral: Choose this if you are a parent/guardian that is currently receiving services from DCFS or another organization (women’s shelter, Dream Center, etc.) They will fill out the form on your behalf.

 

Parental Incapacity: Choose this if you are a parent/guardian that is currently incapacitated and not able to care for your children without assistance. A Licensed Health Professional will fill out the form on your behalf
